What to Expect From Your First Microneedling Appointment

Published by Aestiva | Aesthetic and Skin Clinic in Sidcup

If you’ve been researching microneedling and you’re close to booking — but something is holding you back — you’re not alone. For most people, the hesitation isn’t about whether the treatment works. It’s about not knowing what the experience is actually going to be like.

Will it hurt? Will you walk out looking like a tomato? How much time do you need to take off? What if your skin reacts badly?

These are completely reasonable questions, and this guide answers all of them honestly. By the end, you’ll know exactly what to expect — from the moment you walk through the door to the moment your results fully reveal themselves.

    Before Your Appointment

    The Consultation

At a reputable clinic, your microneedling journey begins before any needles come near your skin. A thorough consultation should take place first — either in advance or on the day — to assess your skin, understand your concerns, discuss your medical history, and confirm that microneedling is appropriate for you.

This is also the moment where your serum will be chosen. As we’ve covered in a previous post, the serum used during microneedling — whether that’s hyaluronic acid, NCTF, or exosomes — is selected based on your individual skin type and goals. It isn’t a one-size-fits-all decision.

If you haven’t already, read our guide: How to Choose the Right Microneedling Serum for Your Skin.

What to do before your appointment

Your practitioner will give you specific pre-treatment instructions, but as a general guide:

    • Avoid retinoids, AHAs, BHAs, and exfoliating acids for at least 5–7 days beforehand
    • Avoid prolonged sun exposure and tanning in the week leading up to your treatment
    • Do not have the treatment if your skin is actively broken out, sunburnt, or irritated
    • Arrive with clean skin — no makeup, SPF, or heavy skincare products on the day
    • Stay well hydrated in the days leading up to your appointment

    Who should not have microneedling?

Microneedling is not suitable for everyone. It is generally contraindicated for those who are pregnant or breastfeeding, have active acne breakouts in the treatment area, have a history of keloid scarring, are on blood-thinning medication, or have certain skin conditions such as eczema or psoriasis in the treatment zone. Your consultation will identify any contraindications before treatment proceeds.

    During Your Appointment

    Step 1: Skin Cleansing

Your skin will be thoroughly cleansed to remove any traces of makeup, oil, or product. This ensures the treatment area is clean and reduces the risk of introducing bacteria into the micro-channels created during needling.

    Step 2: Numbing Cream

A topical anaesthetic cream is applied to the treatment area and left to take effect for approximately 20–30 minutes. This is standard practice at any good clinic and makes a significant difference to comfort levels during the procedure.

Most clients are surprised by how tolerable the treatment is once the numbing cream has taken effect.

    Step 3: The Microneedling Itself

Once the skin is numb, your practitioner will begin the treatment using a medical-grade microneedling device. The device is passed across the skin in a controlled, methodical pattern, creating thousands of microscopic channels in the dermis.

The sensation is often described as a light scratching or vibrating feeling. Certain areas — such as the forehead, nose, and bony prominences — can feel more sensitive than fleshier areas. Most clients find it very manageable and some even drift off during treatment.

The depth of the needles will be adjusted based on your skin concerns and the area being treated. Deeper settings are often used for scarring or more established skin ageing, while lighter settings suit general skin rejuvenation and sensitive skin types.

    Step 4: Serum Application

Throughout the treatment and immediately after, your chosen serum is applied to the skin. The micro-channels allow it to penetrate far deeper than it could topically, which is a core part of what makes the treatment so effective. Your practitioner will work the serum into the skin as they go.

    Step 5: Aftercare Advice

Once the treatment is complete, your practitioner will then walk you through your aftercare instructions in detail.

Total treatment time: Approximately 60–90 minutes including numbing time.

Immediately After — What Will My Skin Look Like After Microneedling?

This is the question most people are most anxious about. Here is an honest answer.

Immediately after microneedling, your skin will look red — similar in appearance to a moderate sunburn. There may be some minor swelling, and the skin may feel warm or tight. Some clients experience very fine pin-point marks where the needles have been, though these are typically not visible from a normal distance.

The intensity of redness varies depending on the needle depth used and your individual skin sensitivity. Lighter, superficial treatments tend to produce mild redness that settles quickly. Deeper treatments for scarring or significant ageing may result in more pronounced redness that takes longer to resolve.

You will not look your best immediately after microneedling. It is worth planning your appointment with this in mind — avoid booking it the day before an important event, a first date, or a work presentation.

    The Microneedling Recovery Timeline

Understanding what to expect day by day will help you feel prepared rather than alarmed.

Day 1 (Treatment day): Redness, warmth, and tightness. Skin looks flushed. Avoid touching your face unnecessarily.

Day 2: Redness begins to subside for most clients. Skin may feel dry or slightly rough. Some people experience very minor flaking. This is normal and is not the same as peeling — do not pick or exfoliate.

Day 3–4: For most clients, visible redness has largely resolved. Skin may still feel slightly sensitised. Some clients notice their skin looks a little dull or flat at this stage — this is temporary and part of the healing process.

Day 5–7: Skin begins to look clearer, more even, and more hydrated. Many clients notice a visible improvement in glow and texture by the end of the first week.

Weeks 2–4: The deeper effects of the treatment — collagen stimulation and structural remodelling — continue to develop. This is where the more significant improvements in firmness, fine lines, and skin quality become apparent.

Most clients are comfortable returning to normal activities within 24–48 hours.

Microneedling Aftercare — What You Must and Must Not Do

Aftercare is non-negotiable. How you look after your skin in the days following treatment has a direct impact on your results and your recovery.

    Do:

    • Keep the skin clean and hydrated with gentle, fragrance-free products
    • Apply SPF 50 every morning without fail — your skin is more vulnerable to UV damage during recovery
    • Use any products recommended by your practitioner
    • Stay hydrated and support your skin from within
    • Sleep on a clean pillowcase

    Do not:

    • Apply makeup for at least 24 hours (ideally 48 hours)
    • Use retinoids, acids, or active skincare for at least 5–7 days post-treatment
    • Exercise heavily, use saunas, or steam rooms for 48 hours — heat increases inflammation
    • Pick, scratch, or exfoliate the skin
    • Have any other facial treatments in the immediate recovery period
    • Apply fake tan or use sun beds

    When Will You See Results?

Results from microneedling are not instant — and this is actually a sign that the treatment is working as it should. The most meaningful improvements come from collagen remodelling, which is a biological process that takes time.

    Here’s a realistic timeline:

    • 1 week: Improved skin texture, hydration, and early glow
    • 4–6 weeks: Noticeable improvement in skin quality, tone, and firmness
    • 3 months: Full collagen remodelling results visible — this is where the real transformation is seen

This is why microneedling is typically recommended as a course of treatments rather than a one-off session. Most clients benefit from 3–6 sessions spaced 4–6 weeks apart, followed by maintenance treatments every 3–6 months to sustain results.

    Frequently Asked Questions

Does microneedling hurt? With numbing cream applied, most clients find the treatment very tolerable. Certain areas feel more sensitive than others, but the majority of clients are surprised by how comfortable the experience is.

Can I wear makeup afterwards? Not immediately. We recommend waiting at least 24–48 hours before applying makeup to allow the micro-channels to close and reduce the risk of irritation or infection.

Will my skin be worse before it gets better? There can be a brief window around days 3–4 where the skin looks a little flat or uneven before it begins to brighten. This is a normal part of the healing process and resolves quickly.

How soon can I go back to work? Most clients feel comfortable returning to work the next day, particularly if they have a desk job and can avoid wearing makeup. If your role involves close client-facing contact and you’d prefer your skin to look completely normal, allow 48 hours.

Can I exercise after microneedling? Light activity is fine, but avoid intense exercise, hot yoga, saunas, or anything that raises body temperature significantly for the first 48 hours.

How do I know if my skin is reacting badly? Some redness, dryness, and sensitivity is completely normal. If you experience significant swelling, pustules, severe pain, or signs of infection beyond the first 48 hours, contact your clinic.

    In Summary

Your first microneedling appointment doesn’t need to be daunting. When you know what’s coming, the experience is straightforward, well-tolerated, and — once you see the results — absolutely worth it.

    • Redness immediately afterwards, settling within 24–48 hours for most clients
    • Skin looking noticeably better within a week, with deeper results over 4–6 weeks
